• Nominees must between 18 and 40 years of age. (as at 1.1.2008). Nominees born before 1 Jan, 1968, are not eligible for The Outstanding Young Malaysian Awards.
  
• The nominee must be a Malaysian citizen. A photocopy of MyKad should be submitted with the Nomination Form to prove the age of the nominee.
  
• Organizing committee members are not allowed to nominate or be nominated. Nominees are allowed to nominate him / herself to participate in The Outstanding Young Malaysian Awards 2008.
  
• The nominee must fill and return the Pre-Entry Form by facsimile or email, as an indication of his/her participation. Fax: 03-9200 5969 Email: jciklm.my@gmail.com OR submit through the online Pre-Entry Form here.
  
• The nominee must sign the nomination form personally. The signature will attest to all facts contained in the form, give permission for publication of the facts and indicate willingness, barring extreme circumstances to attend The Outstanding Young Malaysian Award Ceremony to be held in November if selected for the 2008 The Outstanding Young Malaysian Award.
  
• The nominee must be entered in one – only one – of he ten categories below:

  • Political, Legal and Government Affairs
  • Academic Leadership and Accomplishment
  • Cultural Achievement
  • Moral or Religious Leadership
  • Contribution to Children, World Peace or Human Rights

  • Medical Innovation
    
• A good quality, 4” x 6” (10cm x 15cm) black and white, head to shoulders photograph must be submitted with the nomination form.

• Incomplete forms will be disqualified.
  
• The nominees must be willing to promote ideals of JCI and TOYM for the year 2008/2009 after winning the award.
  
• Nomination Form must reach the Organisers on or before 25^th July 2008 and only in ENGLISH.
