• Nominees must between 18 and 40 years of age. (as at 1.1.2009). Nominees born before January 1, 1969, are not eligible for The Outstanding Young Malaysian Awards. A photocopy of MyKad should be submitted with the Nomination Form to prove the age of the nominee.
• The nominee must be a Malaysian citizen.
• Organizing committee members are not allowed to nominate or be nominated. Nominees are allowed to nominate him / herself to participate in The Outstanding Young Malaysian Awards 2009.
• The nominee must fill and return the Pre-Entry Form by facsimile or email, as an indication of his/her participation.

• The nominee must sign the nomination form personally. The signature will attest to all facts contained in the form, give permission for publication of the facts and indicate willingness, barring extreme circumstances to attend The Outstanding Young Malaysian Award Ceremony to be held in 24 October if selected for the 2009 The Outstanding Young Malaysian Award.
• The nominee must be entered in one – only one – of the ten categories below:

• A good quality, 4” x 6” (10cm x 15cm) black and white, head to shoulders photograph must be submitted with the nomination form.
